What is Medication Therapy Management?

Medication Therapy Management, or MTM, is when a physician refers a patient to an MTM pharmacist (Methodist clinical pharmacist) to provide education on specific disease states (such as diabetes, high blood pressure, high cholesterol, anticoagulation) and to assist with achieving desired targets (blood sugar, blood pressure, cholesterol, INR). The MTM pharmacist is an additional member of your medical care team to help you manage all of your medications, and answer any medication questions, or price concerns you may have.

How do I get referred to MTM?

You can ask your physician for a referral to the MTM pharmacist. The referral MUST be submitted by your physician to the MTM team. The physician will have information on how to submit an MTM referral.

Why should I be referred to MTM?

You will be able to speak directly to a clinical Methodist pharmacist who will assess your therapy and follow-up with you about progress towards your healthcare goals. This may include a discussion of any recent medication changes, or any barriers to taking your medications. The visit will also include discussion of medication side effects you may be experiencing, or any relevant non-medication recommendations, such as dietary recommendations that may benefit your overall health. The MTM pharmacist will coordinate directly with your physician regarding your medications and discuss with your physician any questions or concerns you may have to provide you the best care possible.

Who can be referred to MTM?

ANYONE referred by their physician who wants additional support or education with managing their medications to achieve healthcare goals.

What is the cost of an MTM visit?

There is NO cost to be referred to MTM or to visit with an MTM pharmacist.

How do I meet with the MTM pharmacist?

Visits can be in person at your physician’s office if the pharmacist is on-site, over the phone, or virtually. The pharmacist will coordinate with you on the best option for you.

How often are MTM visits?

The length of the visit and frequency will depend on what concerns need to be addressed. If new medications are started, the pharmacist will likely want to check back with you more frequently to ensure you are doing well.

What can I expect at an MTM visit?

A detailed review of medical history, current medications, medication changes, and medication refills. Close collaboration between pharmacist and referring physician regarding all medications, and access to excellent communication with your healthcare professionals.
